This job description, defining qualifications, primary essential and secondary non-essential functions, responsibilities and duties, is intended as a guideline for the employee and manager. The duties listed here are not considered to be all inclusive. The employee in this position performs these and other duties as required although not listed which may be assigned by his supervisor or facility Administrator/designee
ESSENTIAL FUNCTIONS:
Patient Care Requirements:

Identifies special resident problems and changes in condition of resident and report them immediately to Charge Nurse.

Bathes, neatly dresses (in their own clothing) and undresses residents.
-
Grooms resident including shaving, hair and fingernail care.
-
Accurately records percentage of resident food and fluid intake.
-
Feeds resident according to care plan. Monitors meal tray.
-
Answers call light.
-
Delivers and removes trays.
-
Pulls up side rails and rolls beds down.
-
Changes incontinent residents.
-
Takes residents to bathroom every two hours and as requested.
-
Makes sure all residents are offered nourishment as assigned.
-
Completes tub baths, showers or bed baths.
-
Turns and repositions all bed and chair residents every two hours including pulling up in bed.
-
Empties and records all urinary drainage units.
-
Applies restorative devices such as splints, handrolls, braces, eyeglasses and hearing aides.
-
Utilizes techniques to manage physically and verbally abusive residents.
-
Monitors and redirects confused wandering residents.
-
Assists nurse with treatment procedures.
-
Changes bed linens on occupied and unoccupied beds.
-
Performs oral hygiene and denture care for residents.
-
Takes, records and reports temperature, pulse, respiration rate and blood pressure.
-
Takes, records and reports weights of residents.
-
Accurately uses different types of scales i.e. chair, hoyer, stand-up.
-
Performs routine footcare.
-
Applies creams and lotions to prevent skin breakdown, including back rubs.
-
Inspects resident skin condition with daily care and reports to the charge nurse.
-
Applies restraints correctly and in accordance with residents' care plan.
-
Checks resident who is restrained every hour and releases restraint every two hours.
-
Transfers the resident from bed to chair and chair to bed.
-
Assists resident with use of bedpan or urinal.
-
Provides perineal care to male and female residents.

Safety & Health Requirements:

Identifies safety hazards and emergency situations and reports appropriately.

Follows facility fire, safety and evacuation plans.
-
Functions effectively in emergency situations.
-
Follows facility procedures related to control of equipment and supplies within the unit. Assures that work/assignment areas are clean and equipment and supplies are stored before leaving care area on breaks and at end of shift.

Places bedpans, linen, washbasins and soiled linen in proper area.
-
Observes infection control procedures including hand washing and isolation procedures.

Assumes responsibility for the safety of residents under his/her supervision.
-
Observes all facility safety policies and procedures.
-
Utilizes proper body mechanics for self and residents including use of gait belts and other equipment.
-
Transports residents in required equipment safely to other departments or activities.
-
Maintains residents' units - bedside table, bureau, closets, personal care items in a clean and orderly fashion.

Knows and uses universal precautions.

Documentation Requirements:

Documents incidents and/or unusual problems according to established facility procedures. i.e. personal injuries within 24 hours; residents incidents immediately.

Documents in the nursing assistant notes the care and treatment provided to the resident and the resident's response or lack of response to care provided.

Documents positioning and restraint usage on designated flow sheets.

Demonstrates knowledge and understanding of the impact of disability upon an individual's, as well as families' physical, emotional, social, economic, and vocational status, taking into consideration age-related issues, and developmental levels over the life span (i.e., the adolescents' need to develop their own identity and cope with life changes; and the geriatrics' need to maintain their integrity and independence and adjust to the physiological aspects of aging), and meets competency requirements for the population served (adolescent through geriatric), as applicable to the job description.

Consistently works cooperatively with residents, charge nurses, staff nurses, restorative assistants, other nursing assistants, physicians, families, consultant personnel, ancillary service providers and administration.

Assumes accountability for data contained in the personnel policy manual..

Performs other related duties as directed by licensed nursing personnel.

Accepts assignments to specific areas of nursing home at the discretion of management.

NON-ESSENTIAL FUNCTIONS:

Assists new employees in following established facility policies and procedures.
-
Participates in resident care conferences and other facility meetings as assigned.
-
Reviews care plans and performs nursing care as outlined.
-
Follows facility procedures for admission, discharge, and transfer of residents.
-
Checks name bands on all assigned residents daily and report to Charge Nurse any bands that have been removed or need to be changed.
-
Performs post mortem care according to facility policy and procedure.
-
Accompanies patients to appointments out of facility.
-
May run errands, direct visitors and answer telephone.

Prerequisites:
​

Minimum of tenth grade education. High school diploma or GED preferred.
-
Must be sixteen years of age.
-
Must be certified in state of Massachusetts or Long Term Care (upon hire or within four (4) months of date of hire) eligible for reciprocity.

Overall Strength Factor

Sedentary Work - Exerting up to 10 pounds of force occasionally and/or a negligible amount of force frequently to lift, carry, push, pull or otherwise move objects, including the human body. Sedentary work involves sitting most of the time, but may involve walking or standing for brief periods of time. Jobs are sedentary if walking and standing are required only occasionally and all other sedentary criteria are met.
Light Work - Exerting up to 20 pounds of force occasionally, and/or up to 10 pounds of force frequently, and/or a negligible amount of force constantly to move objects. Physical demand requirements are in excess of those for Sedentary Work. Even though the weight lifted may be only a negligible amount, a job should be rated Light Work: (1) when it requires walking or standing to a significant degree; or (2) when it requires sitting most of the time but entails pushing and/or pulling of arm or leg controls; and/or (3) when the job requires working at a production rate pace entailing the constant pushing and/or pulling of materials even though the weight of those materials is negligible.
Medium Work - Exerting 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly to move objects. Physical Demand requirements are in excess of those for Light Work.
Heavy Work - Exerting 50 to 100 pounds of force occasionally, and/or 25 to 50 pounds of force frequently, and/or 10 to 20 pounds of force constantly to move objects. Physical Demand requirements are in excess of those for Medium Work.
Very Heavy Work - Exerting in excess of 100 pounds of force occasionally, and/or in excess of 50 pounds of force frequently, and/or in excess of 20 pounds of force constantly to move objects. Physical Demand requirements are in excess of those for Heavy Work.

ENVIRONMENTAL REQURIEMENTS

The position of Certified Nursing Assistant includes tasks that involve:
Category I: Exposure to blood, body fluids. or tissues. All procedures or other job related tasks that involve an inherent potential for mucous membrane or skin contact with blood, body fluids or tissues, or a potential for spills or splashes of them, are Category I tasks. Use of appropriate protective measures are requited for every employee engaged in Category I tasks.
Category II: Exposure to blood, body fluids, or tissues, but employment may require the performance of unplanned Category I tasks. The normal work routine involves no exposure to blood, body fluids, or tissues, but exposure or potential exposure may be required as a condition of employment. Appropriate protective measures (i.e. gloves, masks, gowns, etc.) are readily available to every employee engaged in Category II tasks.
Category III: Exposure to blood, body fluids, or tissues and Category I tasks are not a condition of employment. The normal work routine involves no exposure to blood, body fluids, or tissues (although situations can be imagined or reasoned under which anyone, anywhere, might encounter potential exposure to body fluids.) Persons who perform these duties are not called upon to perform or assist in emergency medical care or first aid or to be potentially exposed in some other way as part of their employment. Tasks that involve handling of implements or utensils, use of public or shared bathroom facilities or telephones, and personal contacts such as handshaking are Category III tasks.
